Post to Patient Service

So that patients can continue to receive their lenses during this challenging time, we are offering a ‘Post to Patient Service’. If you would like to order a lens using this service, please fill out the form below and hit send.*

We hereby authorise theses lenses to be posted directly to the above patient. We confirm that the patient is not under 16 or registered sight impaired or severely sight impaired. We confirm that the above specifications are correct and the responsibility for the aftercare of the patient remains with the practice.

*Lenses will be posted to the patient’s address by recorded delivery – No7 cannot guarantee Royal Mail delivery times at present.

*The Post to patient service is temporarily being made available due to the current exceptional circumstances and will be discontinued when normal service can be resumed.

* GDPR statement: The patient’s full name and address details will only be retained by No7 up until the end 90-day warranty period for traceability purposes. Those details will then be removed from our records.
